Indications

Approved Uses

Osteoarthritis and rheumatoid arthritis (adult indications for oral tablets).

Dosage & Administration

Dosing by Condition

Osteoarthritis: 7.5 mg once daily, may increase to 15 mg once daily. Rheumatoid arthritis: 15 mg once daily (some labels allow starting at 7.5 mg and titrating). Ankylosing spondylitis (where approved): 15 mg once daily.

Initial Dose

7.5 mg once daily

Maintenance Dose

7.5mg to 15mg once daily.

Maximum Dose

15mg once daily.

Children's Dosage

Not approved for children under 16 years for this formulation; juvenile idiopathic arthritis dosing (where approved): 0.125 mg/kg once daily, max 7.5 mg/day

Dose Adjustment Notes

Use the lowest effective dose for the shortest duration. In severe renal impairment (including hemodialysis), do not exceed 7.5 mg/day; avoid/use with caution in severe hepatic impairment; use caution in older adults and those at higher GI/renal/CV risk.

How to Take

Swallow the tablet whole with water; take once daily at the same time. May be taken with or without food, but taking with food or milk can reduce gastrointestinal upset.

Side Effects

Common Side Effects

Dyspepsia/indigestion, abdominal pain, nausea, diarrhea, headache, dizziness, peripheral edema, and rash.

Side Effect Frequency

Common (≥1%): dyspepsia, nausea, abdominal pain, diarrhea, headache, dizziness, edema, hypertension, rash/pruritus, and lab abnormalities (e.g., elevated liver enzymes, anemia). Serious but less frequent: GI ulcer/bleeding/perforation, renal impairment/failure, severe cutaneous adverse reactions (SJS/TEN), anaphylaxis, hepatitis, and cardiovascular thrombotic events.

Safety & Warnings

Contraindications

Contraindications: hypersensitivity to meloxicam or excipients; history of asthma/urticaria/other allergic-type reactions after aspirin or other NSAIDs (incl. aspirin-exacerbated respiratory disease); peri-operative pain in CABG surgery. Also contraindicated in late pregnancy (third trimester) per NSAID class labeling.

Warnings & Precautions

Warnings/precautions: use lowest effective dose for shortest duration; monitor for CV events and BP elevation; monitor for GI bleeding (higher risk in elderly/prior ulcer/GI bleed); renal toxicity/AKI risk especially with ACEi/ARB+diuretic, dehydration, CKD; fluid retention/edema/heart failure worsening; severe skin reactions; anaphylactoid reactions in NSAID-sensitive patients; may mask fever/infection; consider hepatic monitoring if symptoms/signs of liver injury.

Age Restriction

Adults and adolescents ≥16 years (this 15 mg tablet strength is not indicated for pediatric JIA).

Drug Interactions

Drug Interactions

Key interactions: anticoagulants (e.g., warfarin/DOACs) and antiplatelets (incl. aspirin) ↑ bleeding; SSRIs/SNRIs ↑ GI bleeding; corticosteroids ↑ GI ulcer/bleed; other NSAIDs ↑ toxicity; ACEi/ARB and diuretics ("triple whammy") ↑ AKI and may blunt BP/diuretic effect; lithium ↑ levels; methotrexate ↑ toxicity; cyclosporine/tacrolimus ↑ nephrotoxicity; cholestyramine ↑ meloxicam clearance.

Interaction Severity

MAJOR/clinically significant: anticoagulants/antiplatelets (e.g., warfarin) and SSRIs/SNRIs (↑ bleeding), other NSAIDs/aspirin (↑ GI toxicity), methotrexate (↑ toxicity), lithium (↑ levels/toxicity). MODERATE: ACE inhibitors/ARBs and diuretics (↑ renal risk/↓ antihypertensive effect), cyclosporine/tacrolimus (↑ nephrotoxicity).

Food Interaction

No clinically significant food restriction; may take with food to improve GI tolerability.

Alcohol Interaction

Avoid

Special Populations

Pregnancy

Pregnancy Category C (1st and 2nd trimesters); contraindicated in 3rd trimester.

Children

Not approved for children under 16 years for this formulation; juvenile idiopathic arthritis dosing (where approved): 0.125 mg/kg once daily, max 7.5 mg/day

Elderly

Use with caution; start at 7.5 mg once daily; monitor renal function, blood pressure, and GI tolerance closely; use lowest effective dose

Kidney Impairment

Mild-moderate renal impairment: no adjustment generally required (use caution). Severe renal impairment: avoid; in hemodialysis, maximum 7.5 mg/day.

Liver Impairment

Mild-moderate hepatic impairment: no dose adjustment (use caution). Severe hepatic impairment: not recommended/avoid.

Storage & Patient Advice

Patient Counseling

Take once daily exactly as prescribed (max 15 mg/day). Take with food/milk if GI upset. Avoid combining with other NSAIDs (e.g., ibuprofen/naproxen) and limit/avoid alcohol. Seek urgent care for GI bleeding (black/tarry stools, vomiting blood), severe abdominal pain, allergic reaction, or serious CV symptoms (chest pain, shortness of breath, sudden weakness/slurred speech). Inform prescriber about anticoagulants/antiplatelets, SSRIs/SNRIs, steroids, ACEI/ARB/diuretics, lithium, and methotrexate. Avoid in pregnancy especially from 20 weeks and contraindicated in 3rd trimester; use lowest effective dose for shortest duration.

Monitoring Requirements

Monitor blood pressure and signs of edema; periodic renal function (SCr/eGFR), liver enzymes, and CBC during prolonged therapy or in high-risk patients; monitor for GI bleeding.

Pharmacology

Mechanism of Action

Inhibits cyclooxygenase (COX) enzymes with preferential inhibition of COX-2 over COX-1, decreasing prostaglandin synthesis to reduce inflammation, pain, and fever.

Duration of Effect

Approximately 24 hours.

Half-Life

15 to 20 hours.

Bioavailability

Approximately 89% (oral).

Metabolism

Hepatic metabolism mainly via CYP2C9 with a minor contribution from CYP3A4 to inactive metabolites.

Excretion

Excreted roughly equally in urine and feces, predominantly as metabolites (about 50% each).

Protein Binding

>99% protein bound (primarily to albumin).
