Indications

Approved Uses

Treatment of the signs and symptoms of allergic conjunctivitis (e.g., ocular itching; may include redness/tearing).

Off-Label Uses

May be used off-label as adjunctive therapy for other allergic ocular conditions (e.g., vernal keratoconjunctivitis/atopic keratoconjunctivitis) when clinician-directed.

Dosage & Administration

Initial Dose

One drop in each affected eye twice daily, 6 to 8 hours apart.

Maintenance Dose

One drop in each affected eye twice daily, 6 to 8 hours apart.

Maximum Dose

1 drop in each affected eye twice daily (maximum: BID per affected eye)

Dose Adjustment Notes

No adjustment needed for renal or hepatic impairment with ophthalmic use due to low systemic absorption.

How to Take

Wash hands. Open one single-dose (unit-dose) container; do not touch the tip. Tilt head back, pull down lower eyelid, instill 1 drop into affected eye(s). Close eye gently and apply nasolacrimal occlusion (press inner corner) for ~1 minute. If using other ophthalmic products, separate by at least 5 minutes. Discard the unit-dose container immediately after use (single use only), even if solution remains.

Transient ocular burning or stinging upon instillation, eye irritation, dry eye, foreign body sensation, headache, blurred vision

Side Effect Frequency

Common (1-10%): headache (~7%), transient ocular burning/stinging, blurred vision, dry eye, foreign body sensation; other reported common events include hyperemia, keratitis, lid edema, pharyngitis, and rhinitis.

Patient Counseling

Use 1 drop in affected eye(s) twice daily. Remove contact lenses before use and wait at least 10 minutes before reinsertion. Do not touch the tip; each unit-dose container is single use-discard after opening. If using other eye drops, separate by ≥5 minutes. Store below 30°C. Seek care for severe eye pain, vision changes, or worsening redness.

Pharmacology

Mechanism of Action

Selective histamine H1-receptor antagonist with mast-cell stabilizing activity, reducing histamine-mediated ocular allergic inflammation (and release of other mediators).

Onset of Action

Rapid-often within minutes (about 3-5 minutes) for relief of ocular itching.

Duration of Effect

Approximately 24 hours for olopatadine 0.77% formulation; for 0.1% formulation, duration is shorter (approximately 8 hours or less based on twice-daily dosing regimen)

Bioavailability

Minimal systemic absorption after ophthalmic use; plasma levels are typically very low/near or below quantification (reported values generally <0.5 ng/mL in studies).
