Indications

Approved Uses

Bacterial conjunctivitis caused by susceptible organisms.

Off-Label Uses

Off-label: bacterial keratitis/corneal ulcer (as part of ophthalmologist-directed therapy) and perioperative prophylaxis in some settings.

Dosage & Administration

Dosing by Condition

Bacterial Conjunctivitis (0.3% solution): Days 1-2: Instill 1 drop in the affected eye(s) every 2 hours while awake (up to 8 times daily). Days 3-7: Instill 1 drop in the affected eye(s) up to 4 times daily while awake.

Initial Dose

Days 1-2: Instill 1 drop in the affected eye(s) every 2 hours while awake, up to 8 times per day.

Maintenance Dose

1 drop in affected eye(s) 2-4 times daily while awake on days 3-7

Maximum Dose

Up to 8 doses (instillations) per affected eye per day during the initial 1-2 days (e.g., q2h while awake), then typically reduced thereafter per prescriber

Children's Dosage

Approved for children 1 year of age and older. Dosing is the same as for adults.

Dose Adjustment Notes

No dose adjustment required for renal or hepatic impairment due to minimal systemic absorption

How to Take

Wash hands. Tilt head back, pull down lower eyelid to form a pocket, instill 1 drop into the conjunctival sac without touching the dropper tip to the eye/skin/surfaces. Close the eye gently and apply nasolacrimal occlusion (press inner corner) for 1-2 minutes. If using other eye meds, separate by 5-10 minutes.

Side Effects

Common Side Effects

Common: transient eye irritation/burning/stinging, conjunctival hyperemia (redness), eye pain, blurred vision, increased tearing, eye discharge, eyelid edema; dysgeusia (unpleasant taste) can occur.

Side Effect Frequency

Common (≥1%): eye irritation/burning/stinging on instillation, worsening of conjunctivitis, eye pain, dysgeusia (taste disturbance). Uncommon (<1%): chemosis, conjunctival hemorrhage, dry eye, eye discharge, eyelid edema, headache, increased lacrimation, keratitis/corneal epithelial disorder, conjunctival hyperemia/red eye, papillary conjunctivitis, blurred vision/reduced visual acuity; rare: hypersensitivity reactions.

Safety & Warnings

Contraindications

Hypersensitivity to gatifloxacin, other fluoroquinolones, or any component of the formulation

Warnings & Precautions

For ophthalmic use only (not for injection/oral use); avoid contaminating dropper tip; avoid contact lenses during active infection; discontinue and seek care if hypersensitivity occurs; prolonged use may cause overgrowth/superinfection (including fungi); if using other eye meds, separate by 5-10 minutes; if formulation contains benzalkonium chloride, it can irritate eyes and discolor soft lenses

Age Restriction

Safety and effectiveness in pediatric patients below the age of 1 year have not been established.

Driving Warning

Safe

Drug Interactions

Drug Interactions

Significant drug interactions are unlikely (minimal systemic absorption); separate from other eye drops by 5-10 minutes

Interaction Severity

No clinically significant drug interactions are expected with ophthalmic gatifloxacin when used as directed.

Special Populations

Pregnancy

Pregnancy Category C: Use only if potential benefit justifies potential risk to fetus.

Breastfeeding

Caution

Children

Approved for children 1 year of age and older. Dosing is the same as for adults.

Elderly

Standard adult dosing.

Kidney Impairment

No adjustment needed.

Liver Impairment

No adjustment needed.

Storage & Patient Advice

Stopping the Medicine

Complete the full prescribed course as directed; do not stop early unless advised due to allergy/serious adverse effects

Patient Counseling

Use exactly as prescribed and complete the course. Wash hands; avoid touching the dropper tip. Instill 1 drop into the lower lid pocket; close eye and press inner corner 1-2 minutes. Avoid contact lenses during active infection; if lenses are permitted by prescriber, remove before dosing and wait at least 15 minutes before reinsertion. Separate from other eye meds by 5-10 minutes. Store below 25°C. Seek care urgently for severe pain, photophobia, vision changes, or worsening/no improvement after 2-3 days. Discard after the in-use period per local labeling/pharmacy policy (commonly 28 days after opening unless the manufacturer states otherwise).

Monitoring Requirements

No routine monitoring required; monitor for signs of superinfection or lack of clinical improvement after 2-3 days

Pharmacology

Mechanism of Action

Inhibits bacterial DNA gyrase (topoisomerase II) and topoisomerase IV, enzymes essential for DNA replication, transcription, repair, and recombination, leading to bacterial cell death

Duration of Effect

Not well-defined as a single 'duration of effect'; dosing is typically multiple times daily to maintain ocular exposure.

Half-Life

Systemic half-life is not clinically meaningful for ophthalmic use due to minimal systemic levels; ocular/tear residence is on the order of ~1-2 hours (reported tear half-life around ~80 minutes).

Bioavailability

Systemic absorption after ophthalmic administration is minimal; serum levels are typically very low/undetectable.

Metabolism

Metabolism is not clinically significant for ophthalmic use because systemic absorption is minimal; when absorbed systemically, gatifloxacin undergoes limited metabolism and is largely excreted unchanged.
