Dosage & Administration

Dosing by Condition

Osteoarthritis: 7.5 mg PO once daily, may increase to 15 mg once daily; Rheumatoid arthritis: 7.5 mg PO once daily, may increase to 15 mg once daily; maximum 15 mg/day.

Dose Adjustment Notes

Use the lowest effective dose for the shortest duration; in hemodialysis, maximum 7.5 mg/day; avoid/use not recommended in severe renal impairment (especially non-dialysis) and use caution in elderly and in hepatic impairment.

How to Take

Swallow the tablet whole with water; may be taken with or without food (take with food/milk if GI upset occurs); take once daily at the same time each day; do not exceed 15 mg/day.

Common Side Effects

Dyspepsia, abdominal pain, nausea, diarrhea, headache, dizziness, peripheral edema; rash can occur.

Side Effect Frequency

Common (≥1% to <10%): dyspepsia, nausea, abdominal pain, diarrhea, constipation, flatulence, headache, dizziness, rash/pruritus, peripheral edema, hypertension, anemia, and upper respiratory tract infection. Uncommon (≥0.1% to <1%): GI ulceration or GI bleeding, elevated liver enzymes, and renal function impairment (including acute kidney injury).

Safety & Warnings

Contraindications

Contraindicated in: hypersensitivity to meloxicam; history of asthma/urticaria/other allergic-type reactions after aspirin or other NSAIDs (AERD); and for peri-operative pain in CABG surgery; additionally avoid/contraindicate in active GI bleeding/peptic ulcer and in late pregnancy (3rd trimester).

Warnings & Precautions

Key warnings/precautions: increased risk of CV thrombotic events; increased risk of serious GI bleeding/ulceration/perforation; avoid use in CABG peri-operative setting; use caution in elderly and in patients with prior ulcer/GI bleed, cardiovascular disease, hypertension, renal impairment, heart failure, dehydration; monitor renal function and blood pressure; may cause serious skin reactions and hypersensitivity; may mask fever/inflammation.

Overdose

Symptoms: GI upset (nausea/vomiting/epigastric pain), drowsiness/lethargy, GI bleeding; severe cases may include hypertension, acute kidney injury, respiratory depression, coma. Management: supportive care, consider activated charcoal if early; cholestyramine can enhance elimination; urgent medical evaluation required.

Patient Counseling

Take SELEKTINE (meloxicam) 15 mg by mouth once daily as prescribed; take with food or milk and use the lowest effective dose for the shortest duration. Do not exceed 15 mg/day and avoid taking other NSAIDs (e.g., ibuprofen, diclofenac, naproxen) or aspirin for pain unless your prescriber advises. Seek urgent care for signs of GI bleeding/ulcer (severe stomach pain, black/tarry stools, vomiting blood), allergic reaction (wheeze, facial swelling, rash), or serious cardiovascular symptoms (chest pain, shortness of breath, sudden weakness/slurred speech). Limit alcohol (increases GI bleeding risk) and inform your clinician if you have a history of ulcers/bleeding, kidney disease, heart failure/hypertension, or if you are pregnant/trying to conceive (avoid especially in the 3rd trimester).

Monitoring Requirements

Monitor blood pressure and signs/symptoms of GI bleeding; check renal function (SCr/eGFR) at baseline and periodically in at-risk or long-term use; consider CBC and liver enzymes with prolonged therapy or if clinically indicated.
