Indications

Approved Uses

Treatment of athlete's foot (tinea pedis), jock itch (tinea cruris), and ringworm (tinea corporis).

Off-Label Uses

Common off-label uses include Candida-associated diaper dermatitis and angular cheilitis (often as part of combination management).

Dosage & Administration

Dosing by Condition

Cutaneous fungal infections: apply to affected area twice daily (or as per label) for about 2-4 weeks and continue for ~1-2 weeks after symptom resolution; tinea versicolor often 1-2 times daily for ~2 weeks. Vulvovaginal candidiasis dosing (e.g., 5 g intravaginally nightly for 7 days) applies only to labeled vaginal cream products, not automatically to this topical tube.

Initial Dose

Apply a small amount (approximately 5 g) to the affected area twice daily

Maintenance Dose

Apply twice daily until clinical cure, typically 2-4 weeks

Maximum Dose

No defined maximum daily dose for topical miconazole 2% cream; apply a thin layer to affected area as directed and avoid excessive/occlusive use unless prescribed.

Children's Dosage

Use in children under 2 years only under medical advice and supervision; no absolute age cutoff for children 2 years and older is specified in the labeling.

How to Take

Topical use: Clean and dry the affected area, apply a thin layer to the affected skin (and a small surrounding margin) and rub in gently; wash hands before and after. Vaginal use applies only if this specific product is supplied with an applicator and labeled for intravaginal use-then insert the prescribed amount intravaginally at bedtime for the labeled duration; otherwise use externally only as directed on the pack.

Pharmacology

Mechanism of Action

Inhibits fungal ergosterol synthesis by inhibiting lanosterol 14-α-demethylase (CYP51), disrupting cell membrane integrity and function.

Duration of Effect

Local antifungal activity is maintained between applications with regular use; clinical improvement is assessed over days, and cure requires completing the full multi-day to multi-week course depending on infection site.

Half-Life

Not clinically applicable for topical miconazole; if systemically absorbed, reported terminal half-life is ~20-24 hours.

Bioavailability

Minimal systemic absorption (<1%) after topical or vaginal application.
