Occasional constipation / rectal bowel cleansing (adults and ≥12 years): 1 bottle (133 mL) rectally as a single dose; do not use more than 1 dose in 24 hours

Contraindicated/avoid in: suspected/known bowel obstruction or ileus, GI perforation, toxic megacolon/megacolon, severe active inflammatory bowel disease/colitis, severe dehydration, clinically significant renal impairment, and hypersensitivity to sodium phosphate; avoid in patients with significant heart failure/ascites or sodium-restricted states unless clinician-directed.

Warnings & Precautions

Key warnings/precautions: do not use more than 1 enema in 24 hours; do not use adult 133 mL in children <12 years; higher risk in older adults and in renal impairment, heart failure, dehydration, or sodium-restricted states; stop and seek care if no bowel movement within ~30 minutes, if rectal bleeding occurs, or if severe abdominal pain/nausea/vomiting develops; avoid use when obstruction/ileus/perforation/megacolon/severe colitis is suspected; avoid prolonged/repeated use (risk of dependence and electrolyte/renal injury).

Kidney Impairment

Pharmacology

Mechanism of Action

Osmotic (saline) laxative: phosphate salts draw water into the colon, increasing intraluminal volume/pressure and stimulating peristalsis to promote evacuation

Onset of Action

Usually 1-5 minutes after rectal administration

Duration of Effect

Typically produces a bowel movement within 1-5 minutes; effect is usually complete within about 30 minutes
