Indications

Approved Uses

Treatment of hyperkeratotic, dry, corticosteroid-responsive inflammatory dermatoses (e.g., psoriasis) where a keratolytic is beneficial.

Dosage & Administration

Dosing by Condition

Hyperkeratotic corticosteroid-responsive dermatoses (e.g., psoriasis): apply thin layer twice daily; reassess if not controlled within 2-3 weeks

Initial Dose

Apply a thin film to the affected area once or twice daily.

Maximum Dose

No fixed maximum grams/day established; apply thin layer twice daily to affected areas for up to 21 days; use smallest amount for shortest duration, avoid large areas/occlusion

Dose Adjustment Notes

No renal/hepatic dose adjustment is required; use the smallest amount for the shortest duration, avoid large surface areas/occlusion, and reduce frequency or discontinue if significant irritation, skin atrophy, or signs of systemic corticosteroid effects occur; stop when control is achieved.

How to Take

Clean and dry affected area, apply thin layer to affected skin twice daily and rub gently; wash hands after (unless treating hands); avoid eyes/mucous membranes; do not use under occlusion unless directed

Application-site burning/stinging, irritation, pruritus, dryness, erythema, peeling; with prolonged/excessive use: skin atrophy, striae, telangiectasia, folliculitis, and secondary infection.

Hypersensitivity to mometasone furoate, salicylic acid, or excipients; untreated bacterial, viral (e.g., herpes simplex, varicella), or fungal skin infections at the application site; rosacea; perioral dermatitis; acne vulgaris; application to ulcerated/broken skin

Avoid combining on the same area with other topical corticosteroids, other keratolytics/exfoliants (e.g., salicylic acid products, retinoids, benzoyl peroxide), or using occlusive dressings unless directed; salicylic acid may increase percutaneous absorption of co-applied topical drugs

Storage & Patient Advice

Missed Dose

Apply when remembered; if it is close to the next dose, skip the missed dose and resume the regular schedule; do not apply extra to make up for a missed dose.

Monitoring Requirements

No routine laboratory monitoring for short courses on limited areas; clinically monitor for local adverse effects (irritation, atrophy, striae, infection) and consider HPA-axis suppression assessment if prolonged use, large surface area use, occlusion, pediatric use, or other risk factors for systemic absorption.

Mechanism of Action

Mometasone furoate is a potent topical corticosteroid that activates glucocorticoid receptors to reduce inflammatory mediator production and vasodilation; salicylic acid is a keratolytic that disrupts corneocyte cohesion to promote desquamation and improve steroid penetration through hyperkeratotic skin.

Metabolism

Mometasone furoate: extensive hepatic metabolism primarily via CYP3A4; salicylic acid: hepatic metabolism mainly by conjugation (glycine to salicyluric acid and glucuronidation).

Protein Binding

Mometasone furoate: high protein binding (~98-99%); salicylic acid: moderate-to-high and concentration-dependent (~50-80%+).
