Indications

Approved Uses

Acute treatment of migraine attacks with or without aura in adults.

Dosage & Administration

Dosing by Condition

initial_dose: 50 mg orally once (some patients may use 25-100 mg based on response/tolerability).
repeat_dose: If symptoms improve then return, may repeat after ≥2 hours.
max_dose_24h: 200 mg in 24 hours.
nonresponse_note: If no relief from the first dose, do not repeat for the same attack.
cluster_headache: Not treated with oral tablets; if used, evidence-based regimen is sumatriptan 6 mg subcutaneously for acute cluster headache.

Initial Dose

50mg to 100mg orally as a single dose at the onset of migraine.

Maintenance Dose

25, 50, or 100 mg as a single dose. If relief, may repeat after 2 hours. Do not exceed 200 mg in 24 hours.

Maximum Dose

200 mg per 24 hours for oral sumatriptan tablets; doses should be separated by at least 2 hours.

Children's Dosage

Oral use is not approved for patients under 18 years. Nasal spray and injection have been used off-label in children and adolescents.

How to Take

Swallow 1 tablet whole with water as soon as migraine headache starts (it can be taken at any time during the attack, but earlier is better). If the headache improves then returns, a second tablet may be taken after at least 2 hours; do not take a second dose for the same attack if the first dose gave no relief. Maximum 200 mg in 24 hours.

Not approved for patients <18 years for oral sumatriptan tablets (use in children/adolescents is generally not recommended).

Storage & Patient Advice

Missed Dose

Not applicable-taken as needed for acute migraine; if a dose is forgotten for an attack, take when needed, but do not exceed the maximum daily dose and do not repeat for the same attack if the first dose provided no relief.

Stopping the Medicine

Safe to stop anytime; no tapering required. Not intended for long-term prophylaxis (use only for acute migraine attacks).

Overdose

Patients have received single oral doses up to 400 mg without serious adverse reactions; monitor for at least 12 hours or while symptoms persist due to elimination half-life of approximately 2.5 hours.

Patient Counseling

Use for acute migraine only (not prevention). Take 1 tablet at headache onset; if it helps then the headache returns, you may take a second tablet after ≥2 hours; if the first tablet gives no relief, do not take another for that same attack. Do not exceed 200 mg in 24 hours and avoid frequent use (risk of medication-overuse headache). Seek urgent care for chest pain, shortness of breath, or stroke-like symptoms; avoid using within 24 hours of ergotamine or another triptan and avoid MAOIs. May cause dizziness/drowsiness-use caution driving. Store below 30°C.

Pharmacology

Mechanism of Action

Selective 5‑HT1B/5‑HT1D receptor agonist that constricts dilated cranial vessels and inhibits trigeminal neurotransmission/neuropeptide release to abort migraine.
