Mechanism of Action

Atorvastatin inhibits HMG-CoA reductase to reduce hepatic cholesterol synthesis and upregulate LDL receptors; ezetimibe inhibits NPC1L1 in the small intestine to reduce dietary/biliary cholesterol absorption.

Onset of Action

LDL-C lowering is typically evident within ~1-2 weeks, with near-maximal effect by ~4 weeks.

Metabolism

Atorvastatin: extensive hepatic metabolism primarily via CYP3A4 to active ortho- and para-hydroxylated metabolites. Ezetimibe: rapidly metabolized in the intestine and liver by glucuronidation to active ezetimibe‑glucuronide with enterohepatic recirculation.
