Indications

Approved Uses

Temporary relief of redness and itching of the eye due to allergic conjunctivitis (e.g., pollen/ragweed/grass/animal dander) and minor eye irritations.

Dosage & Administration

Dosing by Condition

Allergic conjunctivitis/ocular itching and redness: instill 1-2 drops in affected eye(s) up to 4 times daily; do not use longer than 72 hours without medical advice.

Initial Dose

1-2 drops in the affected eye(s).

Maintenance Dose

1-2 drops in the affected eye(s) up to 4 times daily as needed.

Maximum Dose

1-2 drops in the affected eye(s) up to 4 times daily; do not use longer than 72 hours unless directed by a doctor.

Children's Dosage

For children 6 years and older: 1-2 drops in the affected eye(s) up to 4 times daily. For children under 6 years: consult a doctor.

Dose Adjustment Notes

No renal/hepatic dose adjustment is typically required for ophthalmic use; limit use to no more than 4 times daily and generally no longer than 72 hours (3 days) unless advised by a clinician, and use caution in patients with cardiovascular disease, hypertension, hyperthyroidism, diabetes, or narrow-angle glaucoma due to potential systemic sympathomimetic effects.

How to Take

Wash hands. Remove contact lenses before use. Tilt head back, pull down lower eyelid, instill 1-2 drops into the conjunctival sac of affected eye(s). Close eye gently and avoid blinking; do not touch dropper tip to eye/skin/surfaces. Consider nasolacrimal occlusion (press inner corner) for 1-2 minutes to reduce systemic absorption. Wait at least 10 minutes before reinserting contact lenses.

Side Effects

Common Side Effects

Transient burning/stinging, eye irritation, blurred vision, mydriasis (pupil dilation) and possible light sensitivity; rebound redness with overuse/prolonged use.

Side Effect Frequency

Most common: transient ocular burning/stinging/irritation and blurred vision; less common/rare: mydriasis, increased intraocular pressure (especially in narrow-angle glaucoma risk), and rebound hyperemia with prolonged/frequent use; systemic sympathomimetic effects (e.g., hypertension, palpitations, headache) are rare but possible.

Contraindications

Contraindicated in: hypersensitivity to naphazoline, pheniramine (maleate) or any component; narrow-angle glaucoma (or anatomically narrow angles).

Warnings & Precautions

Key warnings/precautions: do not use with narrow-angle glaucoma; do not use >72 hours unless directed; remove contact lenses before use and avoid use for contact-lens-related irritation; avoid contaminating dropper tip; use caution in hypertension/heart disease, hyperthyroidism, diabetes; keep out of reach-accidental ingestion by children can cause severe CNS depression/hypothermia/coma.

Age Restriction

Children <6 years: use only with a doctor’s advice/supervision (not recommended for self-use).

Driving Warning

Use caution when driving or operating machinery; mydriasis (pupil dilation) and blurred vision may occur, which can impair vision and coordination

Drug Interactions

Drug Interactions

Clinically relevant interactions (mainly theoretical/rare with ophthalmic use): MAO inhibitors; tricyclic antidepressants; other sympathomimetics; antihypertensives (including beta-blockers) due to potential additive/antagonistic cardiovascular effects.

Interaction Severity

MAJOR: concomitant MAO inhibitors (and within 14 days of use) may increase risk of hypertensive reactions due to systemic absorption of naphazoline. MODERATE: tricyclic antidepressants and other sympathomimetics may potentiate pressor effects. Other interactions are generally low clinical significance with proper ophthalmic use.

Storage & Patient Advice

Stopping the Medicine

May stop at any time; do not use longer than 72 hours unless directed by a doctor, and stop/seek care if redness/irritation persists or worsens; prolonged/frequent use can cause rebound redness.

Overdose

Ocular overdose: rinse/flush the eye with lukewarm water; ingestion (especially in children) can cause serious systemic toxicity (CNS depression, hypothermia, bradycardia/hypotension, respiratory depression/coma)-seek urgent medical care/poison control.

Patient Counseling

Use 1-2 drops up to 4 times daily; do not use longer than 72 hours (3 days) unless directed by a clinician due to rebound redness risk. Remove contact lenses before use and wait at least 10 minutes before reinserting. Do not touch the dropper tip; do not share. Stop and seek care if eye pain, vision changes, severe/worsening redness, or symptoms persist beyond 72 hours. Consider pressing the inner corner of the eye for 1-2 minutes after dosing to reduce systemic absorption. Follow storage per SFDA: store below 25°C.

Monitoring Requirements

No routine monitoring required for short-term OTC use; monitor intraocular pressure if used in patients at risk for glaucoma

Pharmacology

Mechanism of Action

Naphazoline hydrochloride is an alpha-adrenergic agonist that vasoconstricts conjunctival vessels to reduce redness; pheniramine maleate is an H1-antihistamine that blocks histamine-mediated itching and allergic symptoms.

Bioavailability

Low systemic bioavailability after ophthalmic administration; some systemic absorption can occur via nasolacrimal drainage.
