Indications

Approved Uses

Acne vulgaris.

Dosage & Administration

Dosing by Condition

Acne vulgaris: apply a thin film to affected area(s) once daily in the evening after washing and drying; initial and maintenance dosing are the same, adjusted only for tolerability.

Initial Dose

Apply a thin film to affected areas once daily in the evening.

Maintenance Dose

Apply a thin film to affected areas once daily in the evening.

Maximum Dose

Apply a thin film once daily (typically at night) to affected areas; no systemic maximum dose is established for topical use.

Children's Dosage

Not approved for children under 12 years; for adolescents 12 years and older: same as adult dosing (once daily topical application)

Dose Adjustment Notes

No renal/hepatic dose adjustment is required due to minimal systemic absorption; if excessive local irritation occurs, reduce frequency (e.g., every other day) or temporarily discontinue until tolerated.

How to Take

Cleanse gently and dry the skin; apply a thin film (e.g., pea-sized amount for the whole face) to the affected area(s) once daily in the evening; avoid eyes, lips, nostrils, and mucous membranes; wash hands after application.

Side Effects

Common Side Effects

Erythema, dryness, scaling, burning, stinging, pruritus at application site, skin irritation

Side Effect Frequency

Very common (>10%): application-site dryness, erythema, scaling/desquamation, burning/stinging/irritation. Common (1-10%): pruritus, skin discomfort, sunburn. Uncommon (<1%): contact dermatitis/eczema, eyelid edema, conjunctivitis, rash/skin discoloration, acne flare.

Safety & Warnings

Contraindications

Hypersensitivity to adapalene or any excipients.

Warnings & Precautions

For external use only; avoid contact with eyes, lips, mucous membranes and angles of the nose. Do not apply to cuts/abrasions, eczematous, sunburned, windburned, or otherwise irritated skin. Minimize sun/UV exposure (including sunlamps) and use sunscreen/protective clothing. Expect local irritation early; reduce frequency or discontinue if severe. Avoid waxing/depilatories on treated areas and use caution with other irritating topical products.

Age Restriction

Not approved under 12 years.

Driving Warning

Drug Interactions

Drug Interactions

No clinically significant systemic drug-drug interactions are expected with topical adapalene; avoid or use caution with concurrent topical products that increase irritation (e.g., other retinoids, benzoyl peroxide, sulfur/resorcinol/salicylic acid, medicated/abrasive cleansers, astringents, high-alcohol products).

Interaction Severity

MODERATE: concomitant use with other irritating/drying topical agents (e.g., benzoyl peroxide, salicylic acid, sulfur/resorcinol, abrasive cleansers, astringents/high-alcohol products) may increase irritation; no routine 'major' drug-drug interactions are expected for topical adapalene-use caution with other photosensitizing procedures/agents and emphasize photoprotection.

Food Interaction

No restriction

Caution

Children

Not approved for children under 12 years; for adolescents 12 years and older: same as adult dosing (once daily topical application)

Elderly

Standard adult dosing; no specific adjustments required

Kidney Impairment

No adjustment needed.

Liver Impairment

No adjustment needed.

Storage & Patient Advice

Stopping the Medicine

Can be stopped at any time; no taper is required. Acne may recur after discontinuation.

Overdose

Topical overuse may cause marked local irritation (erythema, peeling, burning/discomfort); manage with supportive skin care and reduce frequency or discontinue until tolerated. Accidental ingestion is unlikely to be life-threatening but warrants medical advice/poison center contact; large/chronic ingestion could cause retinoid (vitamin A-like) toxicity symptoms.

Patient Counseling

Use once nightly on clean, dry skin in a thin layer; expect dryness/peeling and possible early flare in the first weeks; use moisturizer and daily sunscreen/limit sun exposure; avoid eyes/lips/mucosa and avoid applying to broken, sunburned, or eczematous skin; reduce frequency or pause if significant irritation occurs; results usually take 8-12 weeks or longer.

Monitoring Requirements

No routine laboratory monitoring; monitor clinically for efficacy and local tolerability (erythema, dryness, peeling, burning) and adherence.

Pharmacology

Mechanism of Action

Adapalene selectively binds retinoic acid receptors (primarily RAR-β and RAR-γ), modulating gene expression to normalize follicular epithelial differentiation/keratinization, reduce microcomedone formation, and exert anti-inflammatory effects.

Onset of Action

Improvement is typically seen after about 8-12 weeks; maximal benefit may take several months with continued use.

Duration of Effect

Benefit is maintained only with continued use; acne commonly recurs after discontinuation.

Half-Life

Unknown/not well characterized with topical use due to minimal systemic absorption.

Bioavailability

Minimal systemic absorption after topical application; systemic bioavailability is very low/negligible.

Excretion

Primarily biliary excretion with elimination mainly in feces (biliary/fecal) for any absorbed fraction.
