Approved Uses

Bacterial conjunctivitis and bacterial corneal ulcer (keratitis) caused by susceptible organisms.

Off-Label Uses

May be used off-label for perioperative/post-procedural ocular infection prophylaxis when directed by ophthalmology (practice varies).

Dosage & Administration

Dosing by Condition

Bacterial conjunctivitis: Days 1-2: 1-2 drops in affected eye(s) every 2-4 hours while awake; Days 3-7: 1-2 drops four times daily. Bacterial corneal ulcer: Days 1-2: 1-2 drops every 30 minutes while awake and 4 & 6 hours after retiring; Days 3-7: 1-2 drops hourly while awake; Days 8-9: 1-2 drops four times daily (typical total course up to ~14 days; longer only if directed by ophthalmology).

Initial Dose

1-2 drops in the affected eye(s) every 2-4 hours while awake for the first 2 days

Maintenance Dose

1-2 drops in the affected eye(s) four times daily

Children's Dosage

Approved for use in children aged 1 year and older at the same dose as adults

Dose Adjustment Notes

No dose adjustment is required in renal or hepatic impairment for ophthalmic use due to minimal systemic absorption.

Most common: transient ocular burning/stinging/irritation after instillation; other reported effects include redness, itching, tearing, blurred vision/photophobia, foreign-body sensation, and eyelid edema/hypersensitivity (serious allergic reactions are rare).

Half-Life

Systemic ofloxacin elimination half-life is ~4-7 hours; for ophthalmic use the clinically relevant half-life is not well-defined because systemic absorption is minimal.

Metabolism

Ofloxacin undergoes minimal metabolism and is eliminated largely unchanged in urine; after ophthalmic administration, systemic metabolism is negligible due to minimal absorption.
