Indications

Approved Uses

Treatment of signs and symptoms of allergic conjunctivitis (including ocular itching; may include redness and tearing).

Off-Label Uses

Vernal keratoconjunctivitis (VKC) symptom relief may be used off-label in some settings.

Dosage & Administration

Dosing by Condition

Allergic conjunctivitis (0.1% ophthalmic solution): instill 1 drop in each affected eye(s) twice daily, about 6-8 hours apart.

Initial Dose

One drop in the affected eye(s) twice daily (for 0.1% solution).

Maintenance Dose

One drop in the affected eye(s) twice daily (for 0.1% solution).

Maximum Dose

0.1% solution: 1 drop in each affected eye twice daily (approximately 6-8 hours apart).

Children's Dosage

Approved for children 3 years and older: 1 drop in each affected eye twice daily (same as adult dosing)

Dose Adjustment Notes

No dosage adjustment is generally required in renal or hepatic impairment with ophthalmic use due to minimal systemic absorption.

How to Take

Wash hands. Remove contact lenses (especially soft lenses) before use. Tilt head back, pull down lower eyelid to form a pocket, instill 1 drop into the conjunctival sac without touching the dropper tip to the eye/skin/any surface. Close the eye gently for 1-2 minutes and apply gentle pressure to the inner corner (nasolacrimal occlusion). If using other eye drops, separate by at least 5 minutes. Reinsert contact lenses after at least 10-15 minutes.

Side Effects

Common Side Effects

Common: transient burning/stinging or irritation on instillation, blurred vision, dry eye, abnormal/foreign body sensation, headache; dysgeusia (bad taste) can occur.

Side Effect Frequency

Common (1-10%): ocular irritation/burning/stinging, eye pain, dry eye, headache, dysgeusia (bad taste). Uncommon (0.1-1%): blurred vision, keratitis, eyelid edema, ocular hyperemia; other nasopharyngeal symptoms may occur.

Hypersensitivity to olopatadine or any component of the formulation.

Warnings & Precautions

For ophthalmic use only (not oral/injectable); avoid contaminating the dropper tip; may cause temporary blurred vision. Contains benzalkonium chloride-remove soft contact lenses before use and wait about 10-15 minutes before reinsertion; do not use to treat contact lens-related irritation.

Storage & Patient Advice

Missed Dose

Use the missed dose as soon as remembered; if it is close to the next scheduled dose, skip the missed dose and resume the regular schedule-do not double doses.

Overdose

Ocular overdose is unlikely to be clinically significant; rinse the eye with lukewarm water/saline if excess is instilled. If accidentally ingested, seek medical advice/poison center; manage supportively.

Patient Counseling

For ophthalmic use only. Instill 1 drop in affected eye(s) twice daily ~6-8 hours apart. Avoid touching the dropper tip to prevent contamination. Mechanism of Action

Selective histamine H1-receptor antagonist with mast-cell stabilizing activity, reducing histamine and other mediator release and thereby relieving allergic ocular symptoms.

Onset of Action

Rapid onset, typically within minutes (about 5-15 minutes) for relief of ocular itching.

Duration of Effect

Approximately 8 hours per dose (supports twice-daily dosing).

Bioavailability

Low systemic bioavailability after ophthalmic administration; systemic absorption is minimal and plasma concentrations are typically very low/undetectable (reported Cmax generally <0.5 ng/mL).

Excretion

Predominantly renal excretion; a large proportion of an absorbed dose is excreted unchanged in urine (commonly cited ~60-70%).
