Acute treatment of migraine attacks with or without aura in adults.

Off-Label Uses

Off-label examples: acute cluster headache (typically subcutaneous or intranasal formulations), and short-term perimenstrual migraine prophylaxis (intermittent dosing).

Dosage & Administration

Dosing by Condition

Migraine (oral tablet): 50 mg as a single dose (some patients may use 25-100 mg depending on response/tolerability). If the migraine improves but returns, a second dose may be taken ≥2 hours after the first; maximum 200 mg in 24 hours. If there is no response to the first dose, do not repeat for the same attack.

Initial Dose

50mg to 100mg orally at the onset of migraine symptoms.

Maintenance Dose

A second dose of 25-100mg may be taken at least 2 hours after the first dose if the migraine recurs.

Maximum Dose

200 mg per 24 hours (oral tablets).

Children's Dosage

Not approved for children, but used off-label for migraines in children and adolescents.

Dose Adjustment Notes

Hepatic impairment: in mild to moderate hepatic impairment, the maximum single oral dose should not exceed 50 mg; avoid oral sumatriptan in severe hepatic impairment. If 50 mg is ineffective, 100 mg may be tried in a subsequent attack (not as an extra dose for the same non-responding attack).

Common Side Effects

Tingling/numbness (paresthesia), flushing and sensations of warmth/cold, dizziness, drowsiness/fatigue/weakness, nausea/vomiting, and chest/throat/neck/jaw tightness or pressure sensations.

Side Effect Frequency

Common (1-10%): sensations of tingling/heat/flushing, pressure/heaviness/tightness (including chest/throat), dizziness, drowsiness/somnolence, nausea/vomiting, fatigue/weakness; Very rare (<0.01%): minor disturbances in liver function tests.

Safety & Warnings

Contraindications

Ischemic heart disease (including prior MI), coronary vasospasm/Prinzmetal’s angina, history of stroke or TIA, peripheral vascular disease, ischemic bowel disease, uncontrolled hypertension, severe hepatic impairment, hemiplegic or basilar migraine, hypersensitivity to sumatriptan; concomitant use (or within 24 h) of ergotamine/ergot derivatives or another triptan/5‑HT1 agonist; use within 2 weeks of MAO‑A inhibitors.

Warnings & Precautions

For acute treatment only (not prophylaxis) and after migraine diagnosis; assess cardiovascular risk and consider evaluation before first use in patients with risk factors; risk of vasospasm and serious cardiac/cerebrovascular events; avoid in hemiplegic/basilar migraine; medication-overuse headache can occur (avoid frequent use, e.g., >10 days/month); caution with controlled hypertension, hepatic impairment, and history of seizures; consider serotonin syndrome risk with serotonergic drugs.

Missed Dose

Not applicable-used as needed for acute migraine attacks, not on a fixed schedule.

Patient Counseling

Take 50 mg at the start of migraine pain (not for prevention). If you improve then the headache returns, you may repeat 50 mg after at least 2 hours; do not exceed 200 mg in 24 hours. If there is no relief from the first dose, do not take a second dose for the same attack. Seek urgent care for chest pain/tightness, shortness of breath, weakness, or symptoms of stroke; avoid using with other triptans within 24 hours or with ergot medicines within 24 hours, and tell your clinician about antidepressants/other serotonergic drugs. Overuse (e.g., frequent use on many days per month) can worsen headaches.
