GENTACIN 0.3% Eye/Ear Drops (8 ml) is a prescription ophthalmic and otic solution containing gentamicin 0.3%. It is an aminoglycoside antibiotic that irreversibly binds the bacterial 30S ribosomal subunit, causing misreading of mRNA and inhibiting protein synthesis to produce bactericidal effects. It is indicated for treatment of superficial bacterial infections of the external eye (including conjunctivitis, blepharitis/blepharoconjunctivitis and keratitis) and the external ear (otitis externa) caused by gentamicin-susceptible organisms. Supplied as an 8 ml topical solution for ocular and otic use; prescription only.

Indications

Approved Uses

Treatment of superficial bacterial infections of the external eye (e.g., conjunctivitis, blepharitis/blepharoconjunctivitis, keratitis) and external ear (otitis externa) caused by gentamicin-susceptible organisms.

Dosage & Administration

Dosing by Condition

Eye (bacterial conjunctivitis/blepharitis/keratitis): 1-2 drops in affected eye(s) every 4 hours; severe infection: 1-2 drops every hour until improvement, then taper. Ear (otitis externa): 3-4 drops into affected ear 3-4 times daily.

Initial Dose

1-2 drops instilled into the affected eye(s) or ear(s) every 4 hours

Maximum Dose

Ophthalmic: 1-2 drops every 4 hours; in severe infection may be increased to 1-2 drops as often as hourly initially, then reduce as improvement occurs.

Children's Dosage

Same as adult dosing; use with caution in neonates and infants

Dose Adjustment Notes

No renal/hepatic dose adjustment is generally required for topical ophthalmic/otic gentamicin; use caution with prolonged/high-frequency use or large-area exposure due to rare systemic absorption.

How to Take

Eye: Wash hands; remove contact lenses; tilt head back, pull down lower lid to form a pocket; instill prescribed drops without touching the tip; close eye gently 1-2 minutes and apply nasolacrimal occlusion (press inner canthus) for ~1 minute; if using other eye meds, separate by 5-10 minutes. Ear: Wash hands; warm bottle in hand; tilt head/lie with affected ear up; instill drops without touching tip; keep position 2-5 minutes (tragal pumping may help); avoid contaminating dropper.

Side Effects

Common Side Effects

Transient burning/stinging, irritation/redness (conjunctival hyperemia), itching, watery eyes; temporary blurred vision after instillation; eyelid swelling/edema may occur.

Side Effect Frequency

Common: transient burning/stinging, irritation, itching, conjunctival hyperemia/redness (and for ear use, local irritation). Uncommon/rare: hypersensitivity reactions (e.g., eyelid edema/rash), superinfection with prolonged use; corneal epithelial toxicity with prolonged/frequent ocular use.

Safety & Warnings

Contraindications

Hypersensitivity to gentamicin/other aminoglycosides; for ear use: avoid if tympanic membrane is perforated unless specifically directed by a specialist.

Warnings & Precautions

Avoid prolonged use (risk of resistance/superinfection including fungi); avoid contaminating dropper tip; remove contact lenses during active eye infection and avoid soft lenses during treatment; for ear use avoid use with suspected/known tympanic membrane perforation unless directed; possible cross-sensitivity with other aminoglycosides.

Age Restriction

No specific minimum age restriction in labeling; may be used in children (including neonates) only under prescriber supervision.

Drug Interactions

Drug Interactions

No clinically significant interactions expected with topical use; avoid concurrent topical aminoglycosides, and be cautious with systemic aminoglycosides/loop diuretics only if unusual significant systemic absorption is suspected.

Interaction Severity

Generally minimal for topical use; clinically relevant concern is additive ototoxicity/nephrotoxicity only if significant systemic absorption occurs (e.g., prolonged intensive use) or if tympanic membrane is perforated and combined with other ototoxic agents-otherwise interactions are usually minor.

Food Interaction

No food interactions (not applicable for topical ophthalmic/otic use).

Special Populations

Children

Same as adult dosing; use with caution in neonates and infants

Kidney Impairment

No adjustment needed (not applicable for topical ophthalmic/otic use).

Storage & Patient Advice

Missed Dose

Use/apply as soon as remembered; if it is near the next scheduled dose, skip the missed dose and continue the regular schedule; do not double the dose.

Stopping the Medicine

Complete the prescribed course; do not stop early unless significant irritation/allergy occurs-then stop and seek medical advice.

Patient Counseling

Use exactly as prescribed for the eye/ear and complete the full course. Wash hands; avoid touching the dropper tip to the eye/ear/skin or any surface. If using for the eye: remove contact lenses before use and generally avoid wearing them during active infection; if your prescriber allows reinsertion, wait at least 15 minutes after dosing. Seek medical advice if symptoms worsen or you develop significant pain, swelling, rash, reduced vision, or hearing changes. Store below 30°C. Discard 28 days (4 weeks) after first opening.

Monitoring Requirements

No routine lab monitoring is required; monitor clinical response within 48 hours and watch for hypersensitivity/irritation, superinfection (including fungal), and (for ear use) worsening pain/discharge or hearing changes-seek reassessment if no improvement.

Pharmacology

Mechanism of Action

Aminoglycoside antibiotic that irreversibly binds the bacterial 30S ribosomal subunit, causing misreading of mRNA and inhibition of protein synthesis, resulting in bactericidal activity.

Onset of Action

Antibacterial activity begins within minutes after instillation; clinical/symptomatic improvement is typically observed within 24-48 hours.

Duration of Effect

Local antibacterial effect per dose persists for several hours (typically ~4-6 hours), consistent with usual dosing every 4 hours (more frequently in severe infection).

Half-Life

Systemic half-life is 2-3 hours in patients with normal renal function; systemic absorption from topical application is minimal.

Bioavailability

Minimal systemic absorption after ophthalmic or otic use; systemic bioavailability is very low under normal conditions.

Metabolism

Not significantly metabolized (essentially not metabolized).

Excretion

Any systemically absorbed gentamicin is excreted primarily unchanged in urine via glomerular filtration (renal elimination).
