Indications

Approved Uses

Indicated for symptomatic treatment of osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, and acute gouty arthritis; and for short-term treatment of acute pain and primary dysmenorrhea (where locally approved).

Off-Label Uses

Off-label uses may include chronic low back pain and postoperative pain (where not covered by local labeling).

Dosage & Administration

Dosing by Condition

Osteoarthritis: 30 mg once daily (may increase to 60 mg if needed); Rheumatoid arthritis: 90 mg once daily; Ankylosing spondylitis: 90 mg once daily; Acute gouty arthritis: 120 mg once daily for up to 8 days; Acute pain/primary dysmenorrhea: 120 mg once daily for short-term use only.

Initial Dose

120mg once daily for acute pain and acute gouty arthritis

Maintenance Dose

Osteoarthritis: 30-60mg once daily. Rheumatoid Arthritis/Ankylosing Spondylitis: 90mg once daily.

Maximum Dose

Maximum dose is 120 mg orally once daily; 120 mg is generally reserved for acute pain (e.g., acute gout) and should be used for the shortest duration (commonly up to 8 days for acute gout).

How to Take

Swallow the tablet whole with water; take once daily as prescribed; may be taken with or without food (food may delay onset slightly).

Pharmacology

Mechanism of Action

Selectively inhibits cyclooxygenase-2 (COX-2), reducing prostaglandin synthesis mediating pain and inflammation, with relatively minimal COX-1 inhibition.

Onset of Action

Analgesic onset typically within ~30-60 minutes (may be earlier in some patients, especially fasting).

Duration of Effect

Approximately 24 hours (supports once-daily dosing).

Half-Life

Approximately 22 hours.
