Dosage & Administration

Maintenance Dose

Schizophrenia (Adults): 2 to 8 mg per day. Bipolar I Disorder (Adults): 1 to 6 mg per day.

How to Take

Swallow the 2 mg film‑coated tablet whole with water; may be taken with or without food; take once or twice daily exactly as prescribed.

Drug Interactions

Key interactions: CNS depressants (additive sedation); CYP2D6 inhibitors (fluoxetine/paroxetine) ↑ risperidone exposure; enzyme inducers (carbamazepine) ↓ exposure; antihypertensives (additive hypotension); levodopa/dopamine agonists (pharmacodynamic antagonism); QT-prolonging drugs (additive QT risk).

Start at 0.5mg twice daily; increase by 0.5mg twice daily increments; usual effective dose 1-2mg/day. Avoid use in elderly patients with dementia-related psychosis due to increased mortality risk.

Kidney Impairment

Severe renal impairment (CrCl <30 mL/min): start 0.5 mg twice daily; titrate slowly; increases above 1.5 mg twice daily should occur at ≥1-week intervals.

Mechanism of Action

Dopamine D2 and serotonin 5‑HT2A receptor antagonism, with additional antagonism at α1, α2 adrenergic and H1 histamine receptors.

Duration of Effect

Clinical effects may persist for days after stopping due to active metabolite; with regular dosing, symptom control is maintained over 24 hours per dose (once- or twice-daily dosing depending on response/tolerability).

Half-Life

Risperidone t1/2: ~3 hours (CYP2D6 extensive metabolizers) and ~20 hours (poor metabolizers); 9-hydroxyrisperidone t1/2: ~21 hours; active moiety (risperidone + 9-hydroxyrisperidone) effective t1/2: ~20-24 hours.

Bioavailability

Approximately 70% oral bioavailability.

Metabolism

Hepatic metabolism mainly via CYP2D6 to 9-hydroxyrisperidone (paliperidone), with minor contribution from CYP3A4.
