Approved Uses
Reduction of elevated intraocular pressure (IOP) in adult patients with open-angle glaucoma or ocular hypertension who are insufficiently responsive to monotherapy with a prostaglandin analogue or a topical beta‑blocker.
Dosage & Administration
Dosing by Condition
Open-angle glaucoma or ocular hypertension: 1 drop in the affected eye(s) once daily (preferably in the evening); do not use more than once daily.

How to Take
Ocular use: instill 1 drop into the conjunctival sac of the affected eye(s) once daily (preferably in the evening) at the same time each day. Wash hands; avoid touching the dropper tip to the eye/skin. After instillation, close the eye and perform nasolacrimal occlusion (press inner canthus) for ~2 minutes to reduce systemic absorption. If using other eye drops, separate by at least 5 minutes. Remove contact lenses before use and wait at least 15 minutes before reinsertion.

Common Side Effects
Common: ocular hyperemia (redness), eye irritation/burning/stinging, eye pain/discomfort, pruritus, dry eye, blurred vision; prostaglandin-class effects such as eyelash changes and increased iris/periocular pigmentation can occur.

Patient Counseling
Use 1 drop once daily at the same time each day (preferably evening); do not use more than once daily. Wash hands, avoid touching the dropper tip, and perform nasolacrimal occlusion for ~2 minutes. Remove contact lenses and reinsert after ≥15 minutes. Separate from other eye drops by ≥5 minutes. Expect possible redness/irritation and potential permanent iris darkening, eyelash growth, and periocular skin darkening. Seek care for wheeze/shortness of breath, fainting, very slow pulse, or chest pain. Discard the bottle 4 weeks after first opening.
Pharmacology
Mechanism of Action
Travoprost (PGF2α analogue) lowers IOP primarily by increasing aqueous humor outflow (uveoscleral and to some extent trabecular). Timolol (non-selective beta-blocker) lowers IOP by reducing aqueous humor production by the ciliary body.
Duration of Effect
Approximately 24 hours (once-daily IOP control).
Bioavailability
Travoprost is a prodrug rapidly hydrolyzed in the eye to the active acid with very low systemic levels; timolol can be systemically absorbed via conjunctival/nasolacrimal drainage, and nasolacrimal occlusion reduces systemic exposure.
Metabolism
Travoprost is a prodrug hydrolyzed by corneal esterases to the active free acid, then further metabolized (e.g., beta-oxidation and conjugation such as glucuronidation); timolol undergoes hepatic metabolism primarily via CYP2D6.
