Do not use cream for tinea cruris and tinea corporis for more than 2 weeks, and for tinea pedis for more than 4 weeks unless directed by doctor

Side Effects
Common Side Effects
Local application-site reactions: burning, stinging/irritation, erythema/redness, pruritus/itching; sometimes peeling.
Side Effect Frequency
Common: local application-site reactions such as burning/stinging/irritation, erythema and pruritus; Uncommon: contact dermatitis/rash/peeling; Rare/very rare: hypersensitivity reactions (e.g., urticaria, angioedema, anaphylaxis).
Safety & Warnings
Contraindications
Hypersensitivity to clotrimazole or any excipients/components of the cream.
Warnings & Precautions
For external use only; avoid eyes/mucous membranes; discontinue if irritation/sensitization occurs; do not apply to broken skin/open wounds unless directed; avoid occlusive dressings unless advised; if no improvement/worsening after ~2-4 weeks seek reassessment; if used in the genital area, it may temporarily reduce effectiveness of latex condoms/diaphragms.
Age Restriction
Not recommended for children under 2 years unless directed/supervised by a doctor; otherwise can be used in older children and adults per label directions.
Drug Interactions
Drug Interactions
No known clinically significant drug interactions with topical clotrimazole when used as directed; note that vaginal clotrimazole can weaken latex contraceptives, but this is not a typical issue for skin cream use.
Interaction Severity
No clinically significant drug interactions expected with topical clotrimazole; at most, minor additive local irritation with other topical irritants.
Food Interaction
None / not applicable for topical clotrimazole cream.
Special Populations
Children
Safe to use in children; apply thin layer to affected area twice daily. Use with caution in infants under 2 years - consult a physician
Kidney Impairment
No adjustment needed
Storage & Patient Advice
Missed Dose
Apply when remembered; if it is close to the next application, skip the missed dose and continue the regular schedule-do not apply extra to make up.
Stopping the Medicine
Do not stop early; complete the full recommended course (commonly 2-4 weeks depending on site/indication) even if symptoms improve, and reassess if not improving by ~4 weeks.
Overdose
Topical overdose is unlikely to cause systemic toxicity; excessive use may increase local irritation. If accidentally ingested, seek medical advice/poison center-GI upset (e.g., nausea/vomiting/abdominal discomfort) may occur and treatment is supportive.
Patient Counseling
Apply a thin layer to clean, dry affected skin (usually twice daily) and continue for the full recommended duration (often 2-4 weeks; seek advice if not improving). Avoid eyes/mucosa and broken skin; wash hands after use. Stop and seek care if severe irritation/allergy occurs. Note: topical clotrimazole can reduce the effectiveness of latex condoms/diaphragms if used on genital areas.
Monitoring Requirements
No routine monitoring required for topical use; monitor for local hypersensitivity reactions
Pharmacology
Mechanism of Action
Inhibits fungal ergosterol synthesis by inhibiting lanosterol 14-α-demethylase (CYP51), disrupting cell membrane integrity/permeability and inhibiting fungal growth.
Onset of Action
Symptom improvement often begins within a few days; full clinical cure typically requires ~2-4 weeks depending on site/severity (e.g., tinea pedis often longer).
Duration of Effect
Therapeutic effect is maintained only with regular application (commonly twice daily); stopping early increases risk of persistence/recurrence.
Half-Life
Negligible systemic absorption with topical clotrimazole; systemic half-life is not clinically relevant/typically not reported for the cream.
Bioavailability
Minimal systemic absorption through intact skin (generally <1%; often described as negligible).
Excretion
Any absorbed clotrimazole is extensively metabolized in the liver; metabolites are excreted mainly in bile/feces with a smaller renal component.
